This Windows 95 program provides a place to store all your medical facts and histories. All that information that you struggle trying to remember every time go to a new doctor or medical facility and have to fill out their medical information form. Information like medications that you are taking, surgeries and procedures that you may have had, medications that you are allergic to, hospitalizations, your insurance information, immunizations, Social Security numbers of your children and etc. After entering all the medical facts for yourself and the members of your family you can print out a Medical Fact sheet for each individual to take with them when needed or a wallet size printout that they can carry with them at all times. All information can be edited, changed or deleted as the situation of an individual changes. It will make your life a lot easier when you have to deal with the medical profession. In many cases you may find that giving them a Medical Fact Sheet to be put in you file will supplant the necessity of filling out a form.
